VNA of Northwest Indiana
NonprofitDelivering up to two warm, nutritious meals per day, VNA Meals on Wheels helps homebound seniors maintain their independence and their nutritional well-being. For many seniors in our community, a VNA Meals on Wheels volunteer is the only person they will see all day. Knowing that someone cares and having that personal attention enhances quality of life. VNA Meals on Wheels serves a hot, nutritious lunch and optional light supper to the homebound, elderly, and disabled in Porter County. We can meet most dietary needs. We deliver Monday through Friday. Hot lunch costs $5.50 per day, and we have an optional light supper that costs an additional $1.85 per day. Financial assistance is available to those who qualify. Program made possible in part by a grant from the Lake and Porter County Funders Collaborative.

Palliative care helps you retain independence and choice, providing relief from pain, and helping make best days possible. When you or your loved one could benefit from the in-home comfort, care, and coordination that palliative care can provide, consider VNA Hospice NWI—the only palliative care provider in NWI with ACHC Palliative Care Accreditation—and one of only two in the state of Indiana. Dedicated VNA Palliative Care Nurse Practitioners are available to help you with the issues you may face in dealing with a critical illness. In addition to your regular medical treatment, the Palliative Care team members help determine a care plan that meets your wants and needs, including management of pain and other symptoms, counseling and spiritual support, and other therapies. This nationally accredited service is available to anyone facing a serious or life-threatening illness and their family members at their request or at the request of the primary physician.

SHIP offers assistance to senior citizens who are eligible for Medicare and their families and caregivers or those transitioning from Medicaid to Medicare. We can answer questions about Medicare coverage. Direct you to the appropriate agency to sign up for Medicare. Assistance in selecting an appropriate Supplemental Health Insurance plan and Prescription Drug plan. You will need to have the following information / documents available when you call. Our curb to curb service includes a wheelchair accessible fleet of vehicles available 7 am – 5 pm, Monday – Friday. OE’s fleet of vehicles is supported in part by a grant from NIRPC and the Federal Transportation Commission. ? MEDICAID CLIENTS: If your transportation will be billed to Medicaid it is required that you call the centralized dispatch service, Southeastrans at 1-855-325-7588 to schedule a ride. GENERAL PRIVATE PAY REQUESTS: If you need transportation services and are paying directly, you may contact OE Transportation Services directly at 219-464-9621 Ext. 227. ? The rate is $7.50 per one-way trip. As our service is curb to curb, if you require a personal assistant, your attendant can ride free of charge.
